Subacute Inpatient Rehabilitation

Marianjoy provides subacute inpatient care to individuals requiring an extensive hospital stay with a customized therapy plan in our own 20 bed Marianjoy RehabLink™Subacute Center on the third floor of Marianjoy Rehabilitation Hospital in Wheaton. 

Subacute care meets the need of those patients who are unable to withstand the intensity of acute rehabilitation therapy.  It is also ideal for those patients who have completed our acute rehabilitation program, but still need to work toward their functional goals in order to go home.  Responding to these patients needs, Marianjoy has established a new level of care that provides an integrated "link" between acute rehabilitation and home.

The new Marianjoy RehabLink™ Subacute Center is located within Marianjoy Rehabilitation Hospital.  This specialized transitional care center provides comprehensive subacute rehabilitation for patients who require an extensive hospital stay with a customized therapy plan that allows them to move to a higher level of independence with intensive rehabilitation.  This multi-disciplinary program is designed to get individuals back home and performing many of the activities of daily life they engaged in before. 

The Marianjoy RehabLink™Subacute Center is part of the comprehensive continuum of care offered by Marianjoy which also includes acute inpatient rehabilitation, outpatient physical therapy, and physician specialty clinics.

Marianjoy has been a pioneer in the subacute level of care for more than 20 years, working with and establishing partnerships with some of the area’s leading skilled nursing facilities.  As a leader in this level of care, the Marianjoy RehabLink™ Subacute Center is available to provide:

A patient and family-centered care team including nurses, therapists, case manager and physician working together toward the patient’s functional goals

Focused and efficient rehabilitation services that may include physical therapy, occupational therapy and speech therapy

24-hour nursing supervision

Medical management led by a Marianjoy Medical Group board-certified physiatrist— a specialist in physical and rehabilitation medicine

Meetings with the patient, family and the care team to discuss the patient’s condition and progress

Education for the patient and their family to successfully prepare to transition home or to another level of care

Specialized orthopedic protocols

Staffed by a Team of Clinical Experts

The Marianjoy RehabLink™ Subacute Center is staffed by a team of clinical experts including therapists and nurses who work under the direction of a Marianjoy Medical Group physiatrist.  Together with input from the patient and family, the team determines the most effective and efficient treatment plan to help maximize the patient’s independence.  This plan is monitored and adjusted as needed throughout their stay.

The Marianjoy RehabLink™ Subacute Center rehabilitation team includes:

• Board-Certified Physiatrists: Medical management is provided by an on-site Marianjoy Medical Group physiatrist who is involved in assessing and monitoring the patient’s ongoing care and addresses the modification of each patient’s changing needs with the team.
  
  
• Nursing Care: Rehabilitation nurses provide daily nursing care as well as 24-hour nursing supervision.
  
  
• Therapy Team: A team of physical, occupational, and speech-language therapists work together to develop a comprehensive individualized treatment plan to meet the needs and goals of each patient.
  
  
• Case Managers: A case manager is available to coordinate care and discharge planning for the patient.
  
  
• Ongoing Relationship With Referring Physicians: Marianjoy works closely with referring physicians, including orthopedic and neurological physicians, to keep them informed of their patient’s progress.  All care plans and progress reports are shared with the patient’s referring or primary care physician, to whom the patients are returned when they leave Marianjoy.

The Marianjoy RehabLink™ Subacute Center accepts most commercial insurance payments for patients that meet the admission criteria.

Subacute Partnerships

In addition, Marianjoy has partnerships with some of the area’s leading subacute skilled nursing facilities.  Within the scope of subacute care, physicians, nurses, and therapists are available to meet both the medical and rehabilitation needs of all patients.

Marianjoy’s subacute units are partnerships between Marianjoy Rehabilitation Hospital and skilled nursing facilities that provide subacute, or short-stay, medical and rehabilitative care.

Marianjoy has been a pioneer in the subacute level of care. In the early 1980s, Marianjoy piloted subacute care to meet the rehabilitative and restorative needs of the communities we serve. In the 1990s subacute care experienced tremendous growth as hospitals joined their medical and technological expertise with the care and operations of skilled nursing facilities. Recently, Marianjoy has expanded the focus of our partnerships to meet the rehabilitation and medical needs of our subacute patients.

As a leader in this level of care, Marianjoy believes in the power of these partnerships. By combining our superior level of rehabilitation services with facilities known for their skilled nursing and medical care, our subacute sites provide:

A care team including you, your family, nurses, therapists, case manager and physician working together toward your functional goals.

Focused and efficient rehabilitation services that may include physical therapy, occupational therapy and speech therapy.

Four or more hours of nursing care daily, plus 24-hour nursing supervision.

Depending on your situation, care led by a physician: internist, family practitioner, geriatric specialist or physiatrist, a specialist in physical and rehabilitation medicine.

Meetings with you, your family and your care team to discuss your condition and progress.

Education for you and your family to successfully prepare for when you leave the subacute setting.

Our subacute care units see patients who have a variety of diagnoses requiring rehabilitation. Some of these are:

Stroke

Orthopedic conditions such as hip and knee replacements, amputations and multiple traumas

Neurological conditions, such as Parkinson’s disease and Guillain-Barré

Brain injury

Spinal cord injury

We work with a variety of payors including Medicare, third party payors, managed care and private funding.

Attending and consulting physicians are not employed by Marianjoy Rehabilitation Hospital, but are independent physicians with privileges to provide services to patients at our facilities.  The attending physicians are employees of Marianjoy Medical Group, a separate corporation from the hospital.
